Email Confirmation comes from wrong email, not reply-to email

I am testing this form builder, we want whoever fills out the form to receive a copy. But when the customer receives their copy, it comes from [email protected]. I have the correct email set up as sender header, reply-to, etc. We want recipient to know it’s from the company, not spam or end up in spam automatically. Please help!
