Email Before Download

I am trying to use the Email Before Download plug-in, and have installed the Download Monitor and Contact Form 7. I went through the steps to add this to one of our posts, however, after testing it out, there wasn’t a link to the PDF we wanted people to be able to download.

I noticed that it could be because we have the newer version of Download Monitor, however I tried to deactivate that and install the older version, but received a fatal error code.

Any help on this would be great!
