Email and Phone fields disappear from checkout page on plugin activation

Hello, thank you for this plugin.
Unfortunately, it has major issue.
When I activate the plugin – email and phone fields disappear from the checkout billing form! How is that possible? These fields are also not on the plugin page where the form is managed. As soon as I disabled the plugin – fields are back. I’m tested with Twenty Nineteen so it’s not a theme issue.
Second, when I want to reset a settings page, it says “do you want to reset this TAB”. But instead of a tab, it resets all settings at once, with all form changes! And you need to do start from scratch.
There are other issues too, but these are most critical.
