Email and Name Sections Not Showing in IE9

    Hello,

    So far a pretty great plugin, thank you! I was previewing it with Firefox, Chrome and IE9 and everything seems to work except the blocks for the email and name don’t say Email Address, First Name and Last Name. The boxes are blank. Any advice?
