You have to need to customize the plugin otherwise not possible to change fields.
But if you need extra fields. I will update the next version of this plugin. This plugin will provide to default design. You should do your own design.

You changed the labels, You did solve your problem very well.

You can put a new line CSS, like please put z-index of this modal.

I hope it can be solved.
