I’ve noticed that some of the form fields are automatically created

your screenshot shows post meta-fields, so I don’t understand what you are referring to by “automatic form fields”?

Fields like radio buttons and checkboxes are not created.

again, it is unclear what you are referring to.

If I can get the radio button and checkboxes fields to display here

but they are displayed, I am assuming that the first field ‘editor_acceptance_copyright’ is a checkbox on your form. It has been mapped to a custom meta field and that is why you see it here.

BUT the fields are not pulling through the information submitted by the contact 7 form.

most likely you have made a mistake in the way you created the metabox and retrieve the post meta fields from the DB.

You mapped your form field etutor-firstname to the post meta field etutor_firstname and your first screenshot clearly shows that the field has been saved properly. So the issue is with your PHP coding of your metabox.

    You’ll need to either:

    – get help from a developer forum such as Stackoverflow
    – and/or hire a developer to fix it for you.
