E-Pharmacy shop making

Hello Everyone!
I want to make registration of pharmacy shop on my site based on their specific location (by District, by Upazill) and details address.

And I wanna my patient/client will able to find his/her nearby medicine shop by filtering by district and upazill.

And selecting his nearby medicine shop, he will be able to place an order uploading prescription.

How can i do that?
