duplicate sign ups

We are having a recurring issue where one person signs up and it shows twice. This has been happening for a number of months on and off. I’m not sure if this is similar to Contact Form 7, where heavy clickers sometimes submit the data twice.
Additionally, the chair is not always receiving an email of the sign up. Her email is on a few of the sheets and is in addition to the global email in Settings.
Any thoughts on what the issue may be?
