Duplicate record in application form

    Thanks for the great plugin and everything is working fine except the application form .

    I am facing these problems if anyone can help

    1. if i am disabling check-box feature in application form setting then in the front end application form submitted properly but if i will put check-box(for Terms & condition) then its showing an error that please read privacy policy and then submit

    2. when user submitted application form then for some record its working fine but for some records its getting 2 or 3 records for same person.

    3. how can i put validation on the application form field lets assume i am selecting text field for mobile number then where to put the validation code.

    4. Calendar is not working properly , I have created a field for DOB so its showing only 20 years in the years fields, 10 upcoming years and ten last years so if a person is 30 year old he is not able to fill the DOB properly.

    Please help me ASAP
