Duplicate Events for Purchase

Has any one run into an issue with pixel your site pro? I have a stage website not online yet. However, even with a pro plugin the purchase events are duplicated.

I can confirm that there are no other plugins being used for pixel events

FB analytics shows the duplicated events were fired from pixelyoursite

Using the pixel extension I can see that refreshing page does not duplicate the events

I have no additional custom events setup

API events are not enabled

This issue does not happen for all transactions and only some.

How can I start to troubleshoot this issue? Where in the database do we track and mark if the event is fired already and where is the code to handle this. This would be a good start.
