Duplicate Answers

I have noticed recently that I have users with duplicate answers being recorded for some of the radio button based questions. If I go to “View Submissions” then choose one of the results, I will sometimes see a set of user results where, for example, question 4 on gender will say “Are you…?” (user selects male or female from radio buttons) and this line with the question and answer will appear twice or sometimes even three times in that particular result set for that user.
Any suggestions would be welcome. Currently going through the database and removing the duplicates by hand from the user answers table one at a time which is not the most efficient solution.
Maybe a unique index to be put on the user_answers table that joins quiz_id, completion_id and the question_id which would prevent duplicate entries?
