Drop Down Menu Not Working

Hi, I have added a contact form on the page -https://woocommerce-39391-1298319.cloudwaysapps.com/contact

However, the conditional group does not work. The form does not show based on the value chosen in the drop down.

I previously have used CF7 on 2 other sides without this error.

On inspecting, I get a js error but I am not sure how to resolve this.

Hopefully someone can guide me to resolve this.
