I understand that you host “forces” HTTPS and that you do have valid SSL but my colleagues specifically meant the setting in WordPress on “Settings -> General” page.
Both “WordPress Address (URL)” and “Site Address (URL)” options must be set to start with “https://” prefix instead of “https://” there if the site is meant be SSL protected. It’s not that obvious because if there is forced redirect to SSL and valid certificate, site will usually work on HTTPS anyway. But with these options not set to start with “https://” protocol prefix there can be some unexpected issues such as mixed content errors where browser blocks loading of some CSS/JS assets.
So please double check it anyway.
If they are already set to start with “https://”, we’d need to take a closer look as it’s very likely that some other CSS is overriding default one or there’s some unexpected JS conflict. In such case, would you please share direct URL to the page with form in question so we could visit it and take a look?
