Download LOG (some bugs?)

When users download files in field referer i see different results with same files:
<1 https://www.mysite.com/download/
<2 https://www.mysite.com/download/1%20-%20manual_folder/folder/
<3 https://www.mysite.com/download/title.pdf/ (title.pdf but is not name file)
<4 Referer with empty field
why is this happening?
All file download are logged? Are we sure about this?
