    Is it possible to make this “exit confirmation” only in the EXTERNAL LINKS that I have in IMAGES on my website? because I don’t want it to happen when people click on an Adsense ad for example

    Adsense ads are loaded via JavaScript so they are not affected by WP External Links which applies it’s settings server side ??

    Thanks, and “External links” works for example if someone clicks on an image that has a link that takes them to another page. Does it also work with images?

    Yes, you can make links open in a new window both if they are text and images.
