Double Instance of Login Box on Login Page

I was having issues after upgrading TML to 6.3.5 and removed the folder without deactivating it. I replaced the folder and activated it, all else seems to be working well now except I now have two instances of the Login entry box on the Login page. How can I remove the duplicate Login box?
