Double Credit Card fields and 2 place order buttons on checkout page

There are double input fields for each form element on the checkout page. I’m sure it is probably due to a configuration. This includes two place order buttons and only one of them actually works. I need to have the extra form fields and the place order button removed (the one that is actually working). I can share screenshots.
