Double comment forms and Math Spam Plugin issue

    When i click “leave a comment” on the bottom of a post it opens two comment forms. How do I get rid of one?

    Also I installed the Math Comment Spam Protection plugin and I’m hearing it’s showing up without any place to put an answer. I’ve disabled the plugin and will try to reinstall later. Will this fix itself upon re-installation or do I need to add a bit of code?
