Don’t use placeholders as labels, it is bad for accessebility

I just started using your plugin, and I was quite appaled that it isn’t even possible to label input fields in a way that conforms to accessibility standars. Do not use the placeholder attribute as a label. Your plugin doesn’t even provide an option to do it any other way!
