Dont see created forms

Hello,

i have a problem with this plugin, if i create new registration form, shorcode on page works but i can not see this form in list … in All i see list all of my pages, post, custom post but not this form what i created … Under title i see All(4), Published(4) but on list nothing … if i delete something from this list, page or post will be also deleted. Where is problem? Thank you
