dog show entries online form

Hi everyone,
I’m looking for a plugin/form that will do the following:

Allow dog owners to search for their dog in a database (They don’t need to be able to add or edit, just fetch data), which will then populate the fields of an entry form (fields such as owner, handler, kennel club number, DOB etc)

Once they find their dog (by pedigree name, number or owners details), they will select some classes to enter and can submit the form to the show secretary.

The form will then email the details to the secretary, AND email the handler back with payment details for online banking.

I’ve worked with contact forms before, so can generally code those OK, Im not sure about populating the fields from the database though.

Also the site manager needs the ability to maintain the databse, but this can be done via an uploaded file.
