Did you enable like Admin Custom Mode = ON / OFF ?

Go to Dashboard – Settings – Admin Custom Login to enable it.

Thank you, it is now working. I had to log out and in to see the changes.

Why do you think the logo is misplaced?

I think Logo & Social Icon in the correct place.

The only login form is misplaced.
