Does the plugin create random formats??

I have just installed the plugin on several sites and I can’t seem to make a consistent login format:
Sometimes you just enter the custom login, somtimes there is a ? before the custom login and sometimes there is a trailing slash . . . am I missing a setting somewhere that lets me modify these to be a consistent syntax??
Some sites are in the following formats (three examples combine into one image):
https://siteaddress/<choose custom login here>
https://siteaddress/?<choose custom login here>
https://siteaddress/<choose custom login here>/
