Does the magic create a form?

The look and feel I need to tweet is to have customers enter data before they pay for the item.
They select which color item, each the same but with a different cost, put the information in to 3 or 4 unique fields then click add to cart, pay for it and have the information they put in the unique fields emailed to me. (What I need is Contact form 7 and eShop combined)
Does this plugin do that kind of magic? Or am I reading this wrong?
(I’ve been up for 48 hours, I’m looking for unicorns.)
