Does not work on Safari/iphone/ipad

Hi,

The form does not work on Safari on mobile phones or iPads. Will this be fixed anytime soon?
People are not getting my thank you message or a confirmation sent to their e-mail and I am not getting an e-mail either. It takes them straight back to an empty screen.
