Does not retrieve data put in the NAME field

After the form submission, I am not able to see the data that the user put into the NAME field. The automatic email uses email address in the body of the email instead of the user’s name. The notification that the admin receives also shows only email address in both email and name fields. Is there something wrong in my settings?
