Thanks for doing the test and letting us know!
I got a feeling that somehow we are all missing something here, I’m just not yet sure what.
To take things a few steps back, I’ve tested your form that you shared here again on my own test site. I used both Pro version (just to have a confirmation) and the same 1.15.2 free version of the plugin as available here.
I only used “out of the box” setup with no additional plugins enabled and the only changes I made to the form were:
– I updated e-mail addresses so notification would go to my inboxes
– and I set the form to store submission in DB (that was already discussed and you made that change on your end, I believe)
Then I tested registration multiple times and in all cases I had
– “your new account is under review” mail to registering user inbox
– submission notification with activation link to my admin account
– then after clicking on activation link (which, by the way, unfortunately does not give any confirmation but that’s a different story) I got “Account activated” confirmation mail to my user account
– and both user and new sites were created and showing up on “users”/”sites” lists accordingly in Network Admin
Note: since they are not your sites they won’t show up under “My sites” menu, just on “Sites” page in “Network Admin”.
So that appears to be working out of the box and something must be interfering with it specifically on your setup. Yet, you have performed the conflict test without success so I’m wondering:
1. at some point it “almost” worked for you as you stated in this post: “#1. yesterday i got an email application and it included the link:

The account is still not activated and needs your approval. To activate this account, click the link below.(…)”
Later on you mention that you didn’t get activation link any more so I’m wondering what has changed… Do you recall any changes to the site configuration or form configuration that might not been mentioned in this conversation?
This e-mail with a link, was it it after or before you’ve added custom MU plugin code shared by my colleague?
When doing conflict test recently, did you do it with- or without this mu plugin code on site?
