Your website’s theme is causing this issue, to fix it, please add the below code in Appearance > Customize > Additional CSS section or open the form theme that is attached to the exact form and add the code in the Custom CSS section:

.fm-form input[type="text"], .fm-form input[type="email"], .fm-form input[type="password"], .fm-form input[type="url"], .fm-form textarea {
  margin-bottom: 0;
} 

Now please clear website and browser caches and check again.
