Displaying duplicate click to review forms even though one shortcode is used

I am using google chrome and i do not see duplicate click to review forms.

scroll all the way down, i am using chrome too. I added a large buffer so that its not showing up higher. Please scroll down

I see. try turning all of your plugins off but this one to see if it is a conflict with a different plug in. If you still see it when they are all off than you have a problem. If it does go away turn on 1 plugin on at a time and see which one is causing the conflict.

I did it in the past its not a plugin issue but probably the theme issue as when the theme is set to wordpress stock it shows up fine :/ not sure what it could be, the dev is not that quick to reach too..

Humm yeah it sounds like your theme you could try a programer on fiverr.com to see if it is a css isue or a div problem. i have used fiverr for a lot of stuff on my sites. Sorry I could not help ??
