Display Issue – overlapping fields in Event Manager

Hi,
Having a display issue with Events Manager. This page illustrates the problem.
Fields not laying out correctly and overlapping so the form is unusable. Using WP 4.5.2 and Events Manager 5.6.4 so I’m up to date on both.
Any insight as to what the problem could be would be greatly appreciated.
