    Great looking plugin, set it up for the first time tonight and put the shortcode on a page for testing – however the formatting of the form is not inline with the font being used on the site or the spacing as you can see in these screen shots.     There are <pre> and/or <code> tags surrounding the USP shortcode. That can happen when you copy/paste stuff using the WP RTE/Visual Editor/Block. View the shortcode using the plain-text editor/block and remove any surrounding pre/code tags will resolve the issue.
