    Hi Jejj,
    I installed the plugin and setup setting etc, Im having an issue with display in submission form as well as submitted form, also upload image filed is missing in submission form though its set correctly in setting ??

    Glad to help. I’m not sure what might be happening, but one thing that stands out from those screenshots is that the USP shortcode is wrapped with code tags. Take a close look at the shortcode you are using (use the Plain Text editor, not the RTE editor), and remove any code or pre tags from either side of the shortcode. Should help with the display issues.
