Display certain form fields in a table on a page

    I have a form with the following fields:
    Name
    Email
    Phone
    Month
    Membership No
    Comments

    I need to group responses by Month and display in a table with the Month as a header and then Name under each month.

    Is there a way to do this with the shortcode?

    Should I code my own template?

    Or what?
