Display at the starting

I want to display this form at the very starting of the page when opened. When a user fills the form and the form is submitted successfully, then only he will be able to use the following webpage.

It may be done with the hiding of the form when submit button is clicked and form gets submitted successfully. But I don’t know the exact way how to do it.

Please provide me a solution.
