Disappears when using with Gravity Form Widget

I am having a little bit of an issue. So the plugin seems to work fine up to a point. I have it added to all pages, I have it hooked to the footer and I have it set to use the widgets template. Without anything added it shows up fine on all pages, it opens and closes with empty field (nothing has been added) with out a hitch. I am using Gravity form and when I go and add the gravity form widget to it everything seems to stop working. It no longer becomes visible on any of the pages. I am sure there is a conflict somewhere but for the life of me can not figure out where.
Here is the page if you would like to check it out. I will leave it as is enabled with gravity forms added to the widget. You will not see the tab as that is the problem but you can see it all loaded in the page source. Any help would be greatly appreciated.
