Disappearing index page, missing comments file, & headers already sent

I’m having 3 problems. Several months ago my blog was hacked and the index page replaced. Since then I have upgraded & changed the theme.

– My index page keeps disappearing
– My comments file keeps disappearing & I’m getting an error page
– I get a headers already sent error
