disallow_file_edit has to be false

If define( 'DISALLOW_FILE_EDIT', true ); is set in the theme or another plugin, then this plugin does not work and the File Manager link goes to a page saying:

Sorry, you are not allowed to access this page.

Could be an idea to add that to the description of the plugin or check for it in the plugin.
