Deny All rule entered into htaccess

This site has been using the plugin for quite some time, and recently the htaccess has been getting hundreds, if not thousands, of deny rules placed on IP addresses. Not only were there the separate IP blocks, but there was even a blanket ‘deny all’ in the beginning of the htacess this morning.
I checked through the settings and cannot seem to find anything that would have caused this to happen.
I also checked with my hosting company (siteground) to see if any server side issues may have triggered this, and they confirmed that their system did not write any of those rules.
At the moment, I have cleared the htaccess file, deactivated the plugin, and reactivated it again. I reset all my settings and for the moment it appears to be working fine. I would just like to know for future reference what to look for, where to look, to find the cause.
