Deleting my additional fields?

Could you please tell us how these questions are being added? Is it as a select field option?

So it’s under additional fields / then add field / and yes, it’s the select field. There isn’t much an image would show. It adds the select field question just fine, but as soon as I add another select field question and save it, the previous one gets deleted?

I think I just figured it out. I was not updated the names (they were both being called addtional_), so it was deleting one since they were named the same. I did one additional_ and a second addtional1_ and they are both still there!
