If I may add something more… the wording and flow of the process could be improved.
Though you say the action is not reversible, the term “deactivate” implies something different from “delete” or “remove”. It looks as if the info on the account and associated websites will not be removed from your systems (even if there were no purchases that could justify keeping a record). Whether some or all of the info will be kept or not, it would help if it is stated unequivocally.
Regarding the options presented before the deactivation, they seem to be a great way of not loosing users without a good cause. I just think the flow would be much clearer if all options were closed on load, and the Deactivate button appeared only when the last option is selected (eg: I clicked a couple of times on the last option thinking something could happen other than the content being collapsed).
Finally, a single line confirmation message once the account has been deactivated would reassure users everything went as expected.
