Default language link works, other languages 404

  • First of all thanks a lot for your free plugin, it helps me lots of times!

    now it’s not wordking on a website I’m working, so I try in a new wordpress 3.9.2 instalation and nothing happens 404 page. Default language it’s ok, but the rest is 404. I use the last version of qtranslate the plugin.

    Do you know what could it happen?

    After activanting and activating again I solve the 404 problem, but I get this slug:

    slug/?lang=ca
    slug/?lang=ca

    In other websites with your plugin I get:

    es/slug
    ca/slug
    fr/slug

    What am I missing?
