Default Issue image Bug in multisite

    Details:
    WP 4.0
    Multisite with two sites set up, using subfolders.
    For example: domain.com and domain.com/second
    Both sites have IssueM active.

    Bug 1:
    When you go to set a ‘Default Issue Image’ in the IssueM settings of the subfolder site (domain.com/second), once you’ve chosen your image the page refreshes to the IssueM settings of the wrong site (in this case, domain.com).

    If you go back to the IssueM settings page of the subfolder site (domain.com/second) it shows the correct image, and the actions you took seem to have worked correctly as it sets the image correctly for existing issues.

    Bug 2 (which I discovered today):
    However, any additional issues added to the subfolder site (domain.com/second) do not have the default image added correctly.
