Debug Notice messages & wrong label

Hi,

When WP_DEBUG is on, there is a recurring message in the dashboard, just before each check for enabling / disabling each module:

Notice: Undefined offset: 7 in /home/marblava/dev/marblava2/wp-content/plugins/wp-simple-firewall/views/icwp_options_helper.php on line 74

Also there is a wrong label just under “Plugin Configuration Summary”

it says “Login Protection is Off [ Configure Now ]” but it should say “Comments Filter is Off [ Configure Now ]”, because it points to icwp-wpsf-comments_filter page. And the real “Login Protection is Off/On” label is right above.
