Date selection always shows 01/01/1970 when form is submitted

Hey guys,
For some reason the date-picker keeps showing the date 01/01/1970 regardless of date chosen when a form is submitted. I get an email of the submission with all the fields showing the correct information apart from the dates which where picked – which keeps showing 01/01/1970 regardless of what one clicks. I have provided a link below to the form. Not sure what’s causing the problem. All help would be greatly appreciated.
