Date picker won’t appear on contact form

Hi,
I am trying to add the date picker to a Divi contact form following the video tutorial on this site – https://www.peeayecreative.com/how-to-add-a-date-and-time-picker-to-the-divi-contact-form/#:~:text=In%20a%20Divi%20contact%20form,%2C%20that%20isn’t%20important. but it’s not working. I can see the plugin settings have changed a bit since this video, is there something I am missing?
The site I want to add this on isn’t live just yet, goes live in just under 6 hours so I can share a link, but have followed the above tutorial exactly.
Appreciate any help available as the plugin looks perfect for what I am trying to do.
