Date Picker not displaying as option after install

I tried installing this add-on using both the regular plugin install and the include options. After install, the add-on is listed as installed in the “Add-Ons” section under “Custom Fields”, but when I drop down the field types when creating a custom field, it’s not listed as an option. Any suggestions?
