Data request form keeps returning error

I keep getting this error when attempting to submit the data request form:
“Something went wrong while saving the request. Please try again.”
Troubleshooting, but no success…
-Tried deleting the Data Request page, generating a new one and using the shortcode in another page
-Tried different emails
-Disabled caching
-Tried logged-in/logged-out
-Tried cookies accepted/not-accepted
-Browser console shows no errors
-Uninstalling/reinstalling the pluginNot sure what else to do!
