    We recently updated WordPress to 3.9.1 and all was well until two days ago when the dashboard stopped displaying correctly. Here’s a video of what we’re seeing. https://www.youtube.com/watch?v=TvMeBvtCYPg

    As you can see from the video it’s not universally displaying incorrectly.

    Before you make the suggestions I have already disabled all plugins, changed to the default theme, and deleted and re-uploaded all wordpress files and the dashboard still does not display correctly across all pages.

    I should note that the last time any change was made as far as plugins or wordpress updates was made almost a month ago and everything has been working fine until two days ago. I have a feeling that it’s not wordpress itself but rather the javascript that displays the menu for the dashboard.

    If someone could direct me to how I can refresh the files used to display the menu bar I’d greatly appreciate it. Or, If anyone has any other insight as to why it would be doing this.

    Well I answered my own problem. It appears my .htaccess file somehow was corrupted and the permissions were changed (by the webserver somehow).

    I uploaded an old backup copy of my .htaccess file and changed the permissions back to chmod 755 and now the dashboard menus and dashboard are displaying correctly universally.

    If I had waited another five minutes I would not have needed to post this. At any rate I’ll close the issue but hopefully if any others have this same issue this will help them in some way.
