Hi I’m also interested in this. The main thing is adding an email address and birthdate to the form. How do I accomplish adding custom fields?

You need to change from code level for add additional fields. I am planning to make dynamic so that everyone customize form fields from wp admin.

I’m interested in this as well. Mainly just for adding email, but more customization is always nice.
