Custom Theme

I want to change the layout of the lightbox so the description is to the right of the image, rather than under it. From what I understand, I can’t simply add CSS to my WP theme css file, correct? I need to create a custom theme?
Instead of creating a theme from scratch, can I just download the sample custom theme, update the SASS file, zip and add as a plugin?
